To wit: during an open rhinoplasty, a common maneuver is to harvest septal cartilage to make grafts, and among them is a columellar strut to better provide support to the nasal tip. I do not know if the phrase raising the columella is the best one, but a poorly supported tip and be improved by a columellar strut, and the result can simulate a lengthening or straightening of the columella as a result.

Widening of the nasal tip is not typical of standard maneuvers involving the columella. However, if one utilizes a columellar strut graft that is placed or sutured in such a way that it abuts or involves the nasal tip cartilages (the lower lateral cartilages), it is possible the nasal tip could widen. A popular tongue-in-groove columellar strut may be one which, if fashioned too thick, might in theory widen the nasal tip.

All that said a matter of course (typical open rhinoplasty), I would not say that maneuvers to the columella (the incision itself; placement of a strut) for the sure impact the width of the nasal tip, no. In fact, other maneuvers are commonly done in open rhinoplasty to address a €˜boxy or bulbous nasal tip with the use of special trans- or intradermal sutures that REDUCE the nasal tip width to more harmonious, aesthetically ideal dimensions.
